Please note, after the first 3 months, you can maintain Lumen Connect portal access for $90/year (clinic-only pricing).

To ensure you receive thoughtful, personalized care during your designated time and to maintain fairness, any clinical communication outside a scheduled visit (including but not limited to email, medical question, or 10-minute call requiring chart review)

⭐ Self-Pay / Non-Insurance

This is a self-pay program and is not billable to insurance.
All services within the Functional Cardiometabolic Optimization Program — including Lumen Connect integration, data review, monthly metabolic visits, and written plans — are offered outside of insurance coverage.

Your enrollment fee, monthly membership options, and any additional communication fees are patient-responsible charges and cannot be submitted to insurance for reimbursement.

Insurance may still be used for medically necessary diagnostic testing, labs, imaging, or separate cardiology visits that fall outside of this program’s scope.
